Elgin Men's Baseball League Waiver
Read this form carefully and be aware that in signing/accepting these terms & participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims and injuries, damages or losses which you might sustain as a result of participating in any and all activities connected with and associated with this program/activity.

I AM AWARE OF THE COVID-19 PANDEMIC AND RELATED GOVERNMENTAL ORDERS, DIRECTIVES AND GUIDELINES (COLLECTIVELY “DIRECTIVES”), INCLUDING DIRECTIVES FOR FREQUENT HAND WASHING, SOCIAL DISTANCING AND USE OF FACE MASKS IN PUBLIC LOCATIONS. I AM AWARE THAT THESE ACTIVITIES ARE OCCURRING IN A PUBLIC LOCATION DURING THE COVID-19 PANDEMIC. I AM AWARE THAT I COULD BE INFECTED, SERIOUSLY INJURED OR EVEN DIE DUE TO COVID-19 OR DUE TO ACTIVITIES ON THE BASEBALL FIELD. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED AND AGREE TO ASSUME ANY AND ALL RISKS OF BODILY INJURY, DEATH OR PROPERTY DAMAGE, WHETHER THOSE RISKS ARE KNOWN OR UNKNOWN. I WILL NOT HOLD LIABLE AND FULLY RELEASE AND FOREVER DISCHARGE THE ELGIN MEN'S BASEBALL LEAGUE , ITS BOARD MEMBERS AND VOLUNTEERS, ELGIN CONTINENTAL FIELDS BALLPARK, ITS BOARD MEMBERS AND VOLUNTEERS, ELGIN CONTINENTAL LITTLE LEAGE, ITS BOARD MEMBERS AND VOLUNTEERS, THE CITY OF ELGIN, ITS BOARD MEMBERS AND VOLUNTEERS, AND ITS SUBCONTRACTORS

I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages, or losses, regardless of severity, that I may have as a result of participating in the program/activity and will not hold liable and fully release and forever discharge the Elgin Men's Baseball League, its board members and volunteers, Elgin Continental Fields Ballpark, its board members and volunteers, Elgin Continental Little League, its board members and volunteers, Elgin American Little League, its board members and volunteers, the City of Elgin and all its employees and subcontractors. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. In the event of an emergency, I authorize the league board members or officials to secure medical services from any accredited hospital and/or ward and further agree that I will be responsible for any and all medical services rendered.

YO ESTOY CONSCIENTE DE QUE LAS ACTIVIDADES ANTES MENCIONADAS OCURRIRAN EN LUGARES PUBLICOS DURANTE LA PANDEMIA COVID-19, Y QUE POTENCIALMENTE INVOLUCRE ACTIVIDADES EN EL CAMPO DE BEISBOL Y QUE POR LO TANTO SON PELIGROSAS. YO TENGO CONOCIMIENTO QUE PODRÍA RESULTAR INFECTADO, GRAVEMENTE HERIDO Y HASTA MORIR DEBIDO AL COVID-19 O DEBIDO A LAS ACTIVIDADES DE BEISBOL. YO ESTOY PARTICIPANDO VOLUNTARIAMENTE EN ESTAS ACTIVIDADES CON PLENO CONOCIMIENTO DEL PELIGRO QUE CONLLEVAN, Y ESTOY DE ACUERDO EN ASUMIR TODOS Y CADA UNO DE LOS RIESGOS DE SUFRIR UN DAÑO CORPORAL, MORIR O SUFRIR DAÑO A MI PROPIEDAD, INCLUYENDO LOS RIESGOS CONOCIDOS Y LOS RIEGOS DESCONOCIDOS POR MI. No sostendrá responsable y liberar totalmente y para siempre descargar la liga de Elgin Men's Baseball League, sus miembros y voluntarios de mesa, Elgin Continental Little League de sus miembros de la junta y
voluntarios, Continental Fields Ballpark, sus directivos y voluntarios, Elgin Classic Little League, sus directivos y voluntarios, Elgin American Little League, sus directivos y voluntarios, la ciudad de Elgin y de todos sus empleados y subcontratistas.

Reconozco y acepto que hay ciertos riesgos de daño fisico a los
